張家輝扮演的主人公(簡稱:輝)年輕時曾經替江湖兄弟執行殺人任務，被第一天當值警員開槍拘捕，該名警員因此當上英雄，輝則被判終身監禁，但因長期受精神分裂症影響，獲一位前社工兼現任議員相助，坐監三十年後獲假釋。
出獄後，輝已和社會脫節，回到面目全非的旺角，差點連回家也不懂。受精神分裂症影響，他以為永遠也逃不出旺角，就像置身於監獄一樣。在旺角的黑道上，他出手救了來自內地的一對姊妹，其中妹妹是弱智的。 由於輝幫了社團，於江湖上擁有特殊的地位，多次幫忙這對姊妹，卻惹來不少麻煩。 好不容易回到旺角老家，發現已年邁母親患上老人痴呆症，連自己兒子也認不到……有一天，輝母突然清醒與輝相認，卻於當日逝世。
